We are experiencing pain during sex, please help!

Dr Vijayasarathi Ramanathan answers a query from a newly married husband who is finding it difficult to penetrate his wife.

It s been a month that we are married and very happy with each other. The issue we face is having intercourse. We are unable to perform sex. I am not able to penetrate, I feel her vagina is small. I am not forcing it as she may go through more pain and start crying. I weigh 80 kg and she is merely 45 kg, is that a problem? Please suggest what we should do to make it happen! It is very common for a newlywed girl to experience pain and discomfort. What you need to understand is that the vagina is an elastic structure. It can accommodate the head of a baby on delivery so it can accommodate different size penis. This does NOT mean that you can insert your penis against her wish. Weight has got nothing to do in regards to penetration, unless she is feeling uneasy having you lying on top of her. In extreme cases it could be a condition called vaginismus for which a woman needs additional help and support of a professional.

What is vaginismus?

Vaginismus is a painful condition that affects women, mostly during sex. It s the involuntary spasm of the muscles of the lower end of the vagina, something that can make intercourse extremely painful. It happens because instead of relaxing, the muscles tense up. A woman suffering from it can t control when the muscles will spasm. It can happen when she is trying to have sex or when she is having a medical examination like a pap smear. Here’s how vaginismus can make sex painful for women.

Most often, it s caused by physically painful conditions like childbirth, painful intercourse, child sex abuse and molestation and some believe it s the body s natural fear response. The only solution is therapy to overcome the feelings of anxiety. Therapy involves teaching the woman to relax while slowly inserting a gradual series of dilators (lubricated rods) to make her comfortable with the mechanisms of sex. Other methods include exercises like Kegels and pelvic muscle contractions which will help the voluntary response of the vaginal muscles.
